Sports Physicals

Easy. Convenient. Affordable. Bring your rising 6th -12th-grade students in for their FHSAA sports physicals on Monday, July 18th at Providence School. Click here to register. Please refer to the Athletic Resources page for all necessary forms. Bring these forms to sports physical day or if you prefer, have your physician complete the forms prior to the first day of tryouts and give them to the Head Coach or Athletics Office.
